Trip Request Forms

Use this form for online trip requests. You must fill out all information on the form and you will receive an email confirming we have received your request. Please make sure your students names and UIDs are accurate or your trip will be returned for edits. You can monitor the status of your trip request by contacting the school site athletic director secretary.

What if I need to add kids to a trip? Roster Change

You can add up to 15 kids on an already approved trip; you will need the FT number in order to do this. This form must be submitted five days prior to trip date. Use this link to add kids to an approved trip:

Why do I need to submit my trip so far in advance?

Trips require various approvals from site admin to nursing service in order to ensure the safety and well being of our students.

KHSD Board Policy and Administrative Regulations (6153).

Local: 15 days in advance

Out of area or overnight: 20 days in advance

Out of state: 50 days in advance

Trip out of country: Nine months in advance

Can I add students to an already approved trip?

Yes, coming soon will be a form where you can add up to 15 students to an already approved trip.
